Trialist/Guest Player Form

Trialist Information

Teams may add up to five trialists, provided the total number of players does not exceed the maximum roster size for their age group. Trialists must be registered for the current season, come from the same district association, and meet the applicable age, division and tier requirements. Each trialist must be identified on the tournament roster and game sheets. A completed Trialist Consent Form, signed by the player’s current coach and a team official from the tournament team, must be submitted to the referee before every game in which the trialist participates.

Download and complete one Trialist Consent Form for each participating trialist. Please ensure all required signatures are obtained before the tournament. A copy of the completed form must accompany the game sheet for every match in which the trialist plays. Forms may be downloaded using the link below:
